{"id":38502,"date":"2025-03-19T17:35:18","date_gmt":"2025-03-19T21:35:18","guid":{"rendered":"https:\/\/www.hydroassoc.org\/?p=38502"},"modified":"2025-03-19T17:43:35","modified_gmt":"2025-03-19T21:43:35","slug":"types-of-shunt-systems-for-hydrocephalus","status":"publish","type":"post","link":"https:\/\/www.hydroassoc.org\/types-of-shunt-systems-for-hydrocephalus\/","title":{"rendered":"Types of Shunt Systems for Hydrocephalus: What to Know"},"content":{"rendered":"<p><a href=\"https:\/\/www.hydroassoc.org\/shunt-systems\/\">Shunt systems<\/a> help manage hydrocephalus by redirecting excess <a href=\"https:\/\/www.hydroassoc.org\/cerebrospinal-fluid-dynamics-hydrocephalus\/\">cerebrospinal fluid (CSF)<\/a> from the brain to another part of the body, where it can be absorbed. \u00a0While all shunts perform the same basic function, there are different types designed to fit individual patient needs. \u00a0This article explains how each type of shunt system works, when they are used, and how they differ.<\/p>\n<hr \/>\n<h2>Ventriculoperitoneal (VP) Shunt<\/h2>\n<h3>How It Works:<\/h3>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignleft wp-image-33461 \" src=\"https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/08\/HA_VPShunt_ChildMale_web-1-364x600.jpg\" alt=\"\" width=\"179\" height=\"295\" srcset=\"https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/08\/HA_VPShunt_ChildMale_web-1-364x600.jpg 364w, https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/08\/HA_VPShunt_ChildMale_web-1-768x1265.jpg 768w, https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/08\/HA_VPShunt_ChildMale_web-1.jpg 812w\" sizes=\"auto, (max-width: 179px) 100vw, 179px\" \/>A <strong>VP shunt<\/strong> moves CSF from the brain\u2019s ventricles through a catheter that runs under the skin to the peritoneal cavity in the abdomen \u2013 the space outside the intestines\/stomach. \u00a0The peritoneal cavity absorbs the CSF naturally.<\/p>\n<h3>When and Why It\u2019s Used:<\/h3>\n<ul>\n<li>One of the <strong>most commonly placed shunts<\/strong> for hydrocephalus.<\/li>\n<li>Suitable for <strong>patients of all ages<\/strong>, from infants to adults.<\/li>\n<li>Often the first choice because the abdomen provides a large, reliable place to absorb CSF efficiently<\/li>\n<li>Can accommodate <strong>growth in children<\/strong> &#8211; longer tubing can be placed into the cavity and \u2018unwinds\u2019 as the child grows.<\/li>\n<\/ul>\n<hr \/>\n<h2>Ventriculoatrial (VA) Shunt (aka Ventriculojugular \u2013 VJ \u2013 shunt)<\/h2>\n<h3>How It Works:<\/h3>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright wp-image-33868 \" src=\"https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/10\/HA_VAShuntAdult_web_Label-1-433x600.jpg\" alt=\"\" width=\"255\" height=\"353\" srcset=\"https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/10\/HA_VAShuntAdult_web_Label-1-433x600.jpg 433w, https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/10\/HA_VAShuntAdult_web_Label-1-768x1064.jpg 768w, https:\/\/www.hydroassoc.org\/wp-content\/uploads\/2023\/10\/HA_VAShuntAdult_web_Label-1.jpg 799w\" sizes=\"auto, (max-width: 255px) 100vw, 255px\" \/>A <strong>VA shunt<\/strong> moves CSF from the brain\u2019s ventricles through a catheter that runs under the skin to the right atrium of the heart (or sometimes to the large veins draining to the heart \u2013 superior vena cava under the collar bone, jugular vein in the neck), where it is delivered into the bloodstream and naturally absorbed by the body.<\/p>\n<h3>When and Why It\u2019s Used:<\/h3>\n<ul>\n<li>Considered when a VP shunt is not an option, such as when previous surgeries or medical conditions prevent abdominal drainage, or there is a problem of abdominal absorption.<\/li>\n<li>An option for patients who have had multiple abdominal surgeries, and it can be difficult to get the catheter into an open area in the abdomen.<\/li>\n<li>Using the bloodstream provides a consistent way for CSF to be absorbed and does not affect the heart.<\/li>\n<li>More commonly used for older children, teens, or adults, since the catheter in the vein cannot be made too long, and growing children will need the catheter revised and lengthened.<\/li>\n<\/ul>\n<hr \/>\n<h2>Ventriculopleural (VPL) Shunt<\/h2>\n<h3>How It Works:<\/h3>\n<p>A <strong>VPL shunt<\/strong> moves CSF from the brain\u2019s ventricles through a catheter that runs under the skin to the pleural space in the chest, a space between the lungs and the chest wall. The CSF is naturally absorbed by the pleural membranes.<\/p>\n<h3>When and Why It\u2019s Used:<\/h3>\n<ul>\n<li>The pleural space serves as an alternative CSF absorption site.<\/li>\n<li>More commonly used in older children and adults; very small children may not be able to absorb enough CSF by this method.<\/li>\n<li>Considered when VP and VA shunts are not suitable (due to infection, poor absorption, difficulty getting the catheter into the space \u2013 such as from multiple abdominal procedures or lack of veins).<\/li>\n<\/ul>\n<hr \/>\n<h2>Lumboperitoneal (LP) Shunt<\/h2>\n<h3>How It Works:<\/h3>\n<p>An <strong>LP shunt<\/strong> moves CSF from the lumbar spine (the spinal fluid space in the lower back below the end of the spinal cord) through a catheter that runs under the skin to the peritoneal cavity in the abdomen, where it is naturally absorbed by the body. \u00a0Unlike other shunts, it does not drain fluid directly from the brain\u2019s ventricles.<\/p>\n<h3><strong>When and Why It\u2019s Used:<\/strong><\/h3>\n<ul>\n<li>Often used for idiopathic intracranial hypertension (IIH) or syringomyelia.<\/li>\n<li>Can be an option for some cases of communicating hydrocephalus.<\/li>\n<li>May be recommended for patients to avoid a ventricular shunt, avoiding placement through the brain.<\/li>\n<\/ul>\n<hr \/>\n<h2>Comparing the Different Types of Shunts<\/h2>\n<table style=\"border-collapse: collapse;
